Liquid Medication Dispensing:
Opioid Use Disorder (OUD)- Methadone and buprenorphine are the most effective medications for opioid use disorder (MOUD). However, despite liquid methadone displaying greater effectiveness in reducing overdose mortality, improving treatment retention, and being the most dispensed MOUD in economically disadvantaged neighborhoods, it remains the most stigmatized and inaccessible MOUD when compared to buprenorphine/naloxone.
Use in Children- Oral liquids are the preferred formulation for younger children as they are easier to swallow and allow for flexible dosing based on the child’s age and weight. However, when small volumes are required, it can result in over- or underdosing of children.
Use in Elderly- Swallowing difficulties in older adults present challenges for medication management. Crushing solid dosage forms remains controversial as a significant source of medication error and harm. When faced with a patient who is unable to swallow solid dosage forms, considerations for liquids is a common practice, which requires accuracy in measurement.
Critical Needs: Critical barriers in digital health, geography, inaccurate liquid dispensers, and ineffective prescribing practices impede access to effective dispensing of liquid medications, which directly influences the nation’s ability to advance scientific knowledge and improve clinical practice outcomes and safety in OUD, children, and elderly populations. Concerning MOUD management, 75% of assessed OTPs reported capacity concerns, 96% of people had higher rates of dependence than their prescribed MOUD, and daily OTP visits for liquid methadone dispensing are compromised by travel times and negative perceptions.

Medication Management Epidemic: Nonadherence in older adults and people with cognition inhibition (dementia), unilateral arm weakness or paralysis (stroke, amputation), or weakness from aging and movement disorders (elderly, Parkinson's) are associated with increased admissions to nursing homes, poorer health outcomes, results in estimated medication error costs between $100 and $300 billion annually, and is a substantial factor in chronic diseases being a leading cause of death. As the proportion of older individuals with these conditions rise, there is an urgent need for effective management of people to reduce health care costs and improve health outcomes through medication management devices and systems capable of providing in-home support, independence, accurate longitudinal care assessments, and routine remote interventions.
Critical Needs: Despite the existence of over 550 electronic clinical quality measures, advances in artificial intelligence, and a plethora of healthcare analytics companies, there remains an inability to comprehensively improve patient care outcomes as innumerable companies (a) lack interoperability with patients and family members; (b) are unable to track and reduce medication errors in real-time; (c) are incapable of accurately measuring and resolving discrepancies in the care record at granular levels; and (d) function as siloed registries. Rather than relying solely on basic alert systems, SANO's technology transforms the field of medication adherence by developing an interdisciplinary multisensory medication management system. Such evolution is needed today to improve accuracy in measuring adherence in clinical trials, while increasing opportunities to enroll diverse populations, despite proximity of their residence to the study site.
